If this is your first visit with Ohio Orthopaedic Center, you can save time in the reception room by downloading our initial visit forms and filling them out before your appointment. Click on the appropriate form(s) below, print out the form(s) and bring them completed to the receptionist at your first visit.

Billing Information

All of our billing services are conducted on-site from 8:00am – 4:30pm M-F with staff available to answer your questions. We have flexible payment plans available for patients without insurance or with high deductibles. Our electronic billing services are quick and efficient processing our patients claims. We accept a wide variety of insurances including Medicare, Medicaid, BWC, Tricare and most commercial insurances.
